1 – Download The Receipt Template To Record A Salvation Army Donation

You can obtain a blank copy of this receipt by clicking on the buttons or links labeled as a PDF, Word, or ODT file. You may use any of these file versions so long as you have downloaded a one you can view and work with. If you do not have a word processing program or a pdf editor, you can open the PDF version then print it directly from the screen using most up-to-date browsers.

2 – Document Some Basic Information Defining The Receipt

Notice the first two blank lines near the top of the page below the Salvation Army logo. Enter the record number the Salvation Army will use to refer to the concerned donation (and this paperwork) on the blank line labeled "Receipt No." The Salvation Army outlet where the donation is received, and this receipt is issued, must have its "Tax ID Number (EIN)" reported on the next blank line.

3 – Produce A Report On The Donor's Identity

The next area begins with the line labeled "Donated By." Fill in the full name of the Donor on this line. Keep in mind this must be the name of the entity that currently owns the items being donated on this receipt. Thus, if the Donor is a human being, fill in his or her first, middle, and last name whereas if the Donor is a business entity then its full name (including any relevant suffix of status part of its official name) on this line. Once we have reported the identity of the Donor, then we must record the official "Street Address," "City," "State," and "Zip" code where the Donor is located and can be directly contacted.

4 – The Donation Must Be Clearly Presented

The items being donated to the Salvation Army by the Donor above will need to be documented. To begin this process, record the calendar date when the donations were successfully received by the Salvation Army on the blank line labeled as "Donation Date." The next piece of information that must be delivered is the dollar value of the donation being made. It will be up to the Donor to determine what the precise value of the donation is. This value can be determined at the Donor's discretion however, it is recommended that any value reported be determined using the Salvation Army's Donation Value Guide. Once the total dollar value for all the donated items has been determined and summed, record the total value (in USD) on the blank line labeled "Donation Value." The next few empty lines, following the label "Donation Description," will provide an area where you can report each item being donated. While this can be a simple list, it is generally a good idea to itemize each deductible donation and its dollar value. Regardless, make sure to provide a complete inventory of each item this donation consists of. If you do not have enough room, you may either use your editing program to add more lines or you may use this area to refer to an attachment listing the donated items.

5 – A Representative Of The Salvation Army Must Personally Verify This Paperwork

If this receipt is to act as proof that the donated items above have been donated to and received by the Recipient then, this Recipient will have to submit his or her signature to this document. Since the Salvation Army, a 501(c)(3) entity, is the Recipient of this donation, an individual elected by the Salvation Army office where this donation was received must act as the signature party here. This individual will have to locate the blank line labeled "Representative Signature" then sign his or her name on it as proof that the Salvation Army received the items you reported above. Once the Salvation Army Representative has signed the line above, he or she will need to print his or her name on the blank line labeled "Print Name."
